  1. Eligibility: This offer is available exclusively to existing subscribers of Rocket Doctor’s Live Text Chat with a Doctor service (“Eligible Subscribers”). Each Eligible Subscriber is entitled to receive one (1) remote monitoring device (“Device”) upon completion of the designated form.
  2. Promotion Period: The offer is valid while supplies last, within the promotional period specified by Rocket Doctor.
  3. Device Details: The Device is intended for remote health monitoring. It has not been manufactured by Rocket Doctor, and as such, Rocket Doctor does not assert any claims regarding its manufacturing quality or capabilities.
  4. Limitation of Liability: Rocket Doctor will not be liable for any faults, defects, or inaccuracies in the Device or the health data it provides. The use of the Device is at the sole risk of the subscriber.   5. No Exchange or Return: The Device is provided on an “as-is” basis. No returns, exchanges, or refunds are permissible once the Device has been dispatched to the Eligible Subscriber.
  6. Device Usage: Subscribers are responsible for the appropriate use of the Device in accordance with its operating instructions. Rocket Doctor is not responsible for any harm or injury resulting from misuse or malfunction of the Device.
  7. Changes to Terms and Conditions: Rocket Doctor reserves the right to modify these terms and conditions at any time without prior notice. Continued participation in this promotion after any such changes constitutes acceptance of the new terms.
